1. If you haven't had a long term care assessment, contact your County public health nurse.

2.  Get your maximum budget amount from your County case manager, health plan coordinator or tribal case manager.

3.  Make a Community Support Plan using the Forms on this site or hire a Flexible Case
     Manager using the
Service Provider link on this site.

4.  Select a Fiscal Support Entity (the organization that pays your bills for this program)
      by using the
Service Provider link on this site.

5.  Submit your Community Supports Plan to your County Case Manager.

6.  Find and hire who you want to work for you and enroll them with your Fiscal Support Entity.

7.  Submit your bills and timesheets to your Fiscal Support Entity.
